What you need for your application
To help you renew quickly, please have the following ready:
- Proof of citizenship or immigration status: If your child or family's citizenship status has changed since last year, you may need to submit this to New York State of Health when you renew.
- Proof of income: You may need to provide proof of income (pay stubs, child support/alimony, unemployment insurance, etc.). If your income is the same as last year, you can use your Social Security number as proof.
